MR images of 64-year-old woman with acute vertigo and history of hypertension and diabetes. The image in the lower right corner, is the threshholded MR angiogram (white) superimposed on the T1-weighted/FLAIR/ T2-weighted RGB composite. Note the infarction of the left posterior inferior cerebellar artery (PICA) (yellow arrow). The MR angiogram (MRA) demonstrates absence of flow in the left vertebral artery (green arrow) and PICA. The red arrow indicates a patent right vertebral artery.
More cephalad section through posterior fossa demonstrates left cerebellar hemispheric PICA territory infarction (arrow). Diffusion-weighted images and ADC maps, not shown, demonstrated restricted diffusion, indicative of acute infarction.
